Enhanced Care Management (ECM)
What is Enhanced Care Management (ECM)?
“ECM” is a new program that offers extra support to eligible members who face health complications and social barriers that prevent them from maintaining their health.
As a member, you may receive services such as:
- Connecting with your primary care doctor, specialists, and other providers.
- Getting help coordinating transportation to your medical appointments.
- Assistance with your support system, including your caseworker, to get the best medical care.
- Education and access to your medications and help navigating new prescriptions.
- Information and referrals to community resources like food banks, housing, job workshops, etc.
- Support/assistance for your family and caregivers.

Who Will Help Me?
You will be assigned an ECM coordinator/caseworker who will help you navigate the system and provide support in your health journey:
- We will establish monthly check-ins to improve your health.
- We will create goals to improve and enhance your quality of life.
- We will keep your doctors informed about changes in your health.
- We will coordinate care and specialist visits in a simple and effective way.
- We will provide support and resources to your family and support team.

How Do I Qualify?
To be eligible for ECM services, members must:
- Be enrolled in the Medi-Cal program.
- And meet one or more of the following conditions:
- Be at risk of or experiencing homelessness, health complications, emotional disorders, or physical problems.
- Be at high risk of mental health disorders, substance use, and social factors.
- Frequently visit the emergency room, have avoidable hospital or recovery center admissions.

After enrollment, your worker will contact you to complete a brief assessment and learn about your needs. A care plan will be created specifically for you.
