Northeast Community Clinic’s PACE Vision

Northeast Community Clinic is preparing for an important new chapter in our mission to provide accessible, compassionate, and culturally responsive healthcare to the communities we serve.

Our future vision includes breaking ground on a new Program of All-Inclusive Care for the Elderly, or PACE, our state of the art campus in the City of Bell Gardens. This project will help expand access to coordinated healthcare and supportive services for older adults who want to remain healthy, independent, and connected to their families and communities.

The Bell Gardens PACE campus represents more than a new building. It represents a long-term investment in seniors, caregivers, and underserved communities throughout Los Angeles County.

A Comprehensive Approach to Senior Healthcare

PACE is designed to help eligible older adults receive coordinated medical care, social services, rehabilitation, transportation, nutrition support, and other essential services through one integrated program.

By bringing services together, PACE can help seniors:

  • Remain safely in their homes and communities
  • Receive coordinated medical and specialty care
  • Access transportation and supportive services
  • Reduce avoidable emergency room visits and hospitalizations
  • Maintain greater independence and quality of life
  • Receive care that respects their language, culture, and individual needs

For families and caregivers, PACE can also provide greater coordination, support, and peace of mind.

Our Bell Gardens PACE Campus

Northeast Community Clinic’s planned Bell Gardens PACE campus will create a welcoming and comprehensive center where seniors can receive healthcare, participate in social activities, access rehabilitation services, and build meaningful connections.

The campus is expected to include space for:

  • Primary and preventive medical care
  • Rehabilitation and therapy services
  • Social and recreational activities
  • Nutrition and meal services
  • Care coordination and supportive services
  • Transportation assistance
  • Pharmacy and other integrated healthcare resources

Our goal is to create an environment that promotes dignity, independence, wellness, and community.

Expanding Access Beyond Bell Gardens

The Bell Gardens campus is the first step in a broader vision.

Following the successful development of this location, Northeast Community Clinic’s long-term goal is to establish two additional PACE campuses in communities where seniors face significant barriers to coordinated healthcare and supportive services.

This expansion would allow us to reach more older adults, strengthen the healthcare safety net, and bring comprehensive senior care closer to the families who need it most.

Each future PACE program will be guided by community needs, local partnerships, accessibility, and Northeast Community Clinic’s longstanding commitment to serving low-income, medically underserved, and culturally diverse populations.

Investing in Our Seniors and Our Communities

Northeast Community Clinic is currently undertaking a capital fundraising effort to help advance the Bell Gardens PACE project and lay the foundation for future expansion.

Capital support will help fund critical project needs, including:

  • Campus development and construction
  • Medical and rehabilitation equipment
  • Technology and care-coordination systems
  • Transportation resources
  • Senior activity and community spaces
  • Furnishings and program infrastructure
  • Future PACE planning and expansion

Support from philanthropic partners, foundations, corporations, community leaders, and individual donors will be essential to turning this vision into reality.
